I changed the name of my site and it is still reflecting the old name plus the new name. How do I totally remove the old name?

The address https://theunbreakableballerina.wordpress.com/ was deleted, so I presume you changed the site address to the present one, https://coachtamarakellyblog.wordpress.com/.
The site title is still “theunbreakableballerinablog.” The site title and the tagline (“Becoming a Ballerina”) may be changed at the General Settings page. See the General Settings support page for information and instructions.
Also, the title “The UNBREAKABLE Ballerina” appears at the top of the front page, because
1. Your front page displays posts in reverse chronological order, and
2. Your latest post is titled “The UNBREAKABLE Ballerina.”The website address http://theunbreakableballerina.wordpress.com (despite being deleted) still appears on your forum profile page, because it hasn’t been changed yet at Account Settings > Web Address.

If you should want to also change the username, theunbreakableballerina, you may do so. See the Change your Username support page.

Domain name changes usually take few hours to propagate fully. In the transition time its likely you would see a cached version of the old link.
You may want to check the domain settings of your blog to see if the old name still appears there.

Sorry, no, restoring the deleted site is not possible. At least not for two years from the date the site was deleted. See the Recycling Blog Addresses support page, which begins
For the most part, we cannot recycle or reassign site addresses (URLs) which may appear to be abandoned. We respect bloggers’ ownership of their content and therefore will not move or delete a site without its owners consent — no matter how long it has been since their last update.
Sites already deleted by their authors two or more years ago may become available again, if they present no username or other technical conflicts.

I should have omitted most the first part of that quoted passage, which doesn’t pertain to your case. The relevant part is as follows:

I know by experience that this includes sites deleted by their owners who later, no matter how soon after the deletion, change their mind and would like to have the address/URL restored.
