someone's blog uses my picture without my permission

Someone has a blog on WordPress that uses my photo, links to my blog and false statements about me without my permission. Their blog is inflammatory and libelous. I do not want to be associated with this blog or on it.
I’ve commented on the blog asking that all mention of me and my photo be removed, and I’ve been ignored.
I found the blogger on Twitter and asked her to remove me, but she refuses to do so and has now been harassing me relentlessly for 5 days. Her name is Janeal Ellers.
The blog I need help with is: (visible only to logged in users)

i want this blog shut down permanently and I want the owner to be banned for life from wordpress.

To determine whether or not the blog is hosted at WordPress.com.
If you see a link that reads “Blog at WordPress.com” in the footer of the blog (linked to http://wordpress.com), this normally means that it is hosted here (although external blogs may place this in the footer, if they choose to).
