Special code for form then submits to email

    I am looking for a plugin/widget that would allow me to import an excel document of contacts that generates a special code so each person has their own code to enter into a one line form. Once they have entered in the special code, it will send an email to me letting me know who has entered in the code.
    Is there anything like that out there. Any direction would be helpful.
