Strange prompts on my self-hosted blog

Good morning,
My self-hosted blog at mattclarkson.name is suddenly asking me to “connect,” which is odd because the site has been connected to WordPress.com for some time now and Jetpack has been working fine for months. Here’s the message I’m seeing —
“Almost done. Before WordPress.com Connect can finish activating, you’ll need to register your website as an application on WordPress.com”
When I click that link, I’m directed to Settings > “WordPress.com Connect”. I’m not sure what to enter for “WPCC Client ID” and “WPCC Client Secret”. I even logged into WordPress.com and couldn’t find the information I need anywhere.
This is perplexing because the blog has been linked with WordPress.com for some time now and Jetpack has been functioning just fine all this time.
