Structure theme – customisation

Hello- am trying to customise the theme – Structure and cannot see how to add an image to the top section, just under where the title is displayed? (next to the text dropped in (rhs widget) I want to get rid of the image on rhs which can do easily but how can I get an image in there to it’s left-hand side- the larger one where there seems to be a space too?
Should it automatically go in there- is it the blogs featured image? Is there something I should be switching on?
