Submission Form Plugins

  I would like to create a submission form. I found this page: http://wordpress.org/plugins/form-maker/installation/

  But I can’t find the plugin menu. How can I add submission forms to my blog?

  The blog I need help with is: (visible only to logged in users)

  While plugins are common on self hosted WordPress sites, due to security concerns, they are not available on WordPress.com. There is more info regarding WordPress.com and plugins here:
