I need a submition form that is similiar to this one at this url:

is there any other forms maybe outside wp?
the reason i ask is because i need about 5 or 6 bars for info…
if not i can make it work i guess- but do you know of any others?

There are no other forms that I am aware of that can be used on wordpress.com blogs. This is a multi-user blogging platform and the code used for forms, iframes, javascript, etc. will be stripped out by the software to preserve security. See > http://en.support.wordpress.com/code/

You can’t edit the form itself. But as I said above, add a picture or logo above the form. Add text to let your readers know what information you want from them.
To put the form on your page, just type in the shortcode:
