  HI, I would like to add a page on my side bar for people to request my complimentary wellness guide through email. Something along the line of “click here to receive my complimentary wellness guide? The click here would be a link for my email. Would I just use a text box for this?

  Thank you, one more quick question, I have the form now but it is just a blank box, do I need to do something else?

  When you are logged in, all you will see is just the text area since wordpress knows who you are. Log out of wordpress and go to the page and then you will see the email, name and website fields.

  Also, if you have not, turn comments off on that page so that there aren’t two text areas on the page.
