Provided you have installed and activated the JetPack plugin http://jetpack.me on your WordPress.org install, and posted the relevant URL(s) into this forum thread, Staff will do the transfer of subscribers and stats for you. I tagged this thread for their attention.

Hi swaroop,
Your WordPress.com subscribers and email followers have been transferred as requested.
Please note that any WordPress.com followers who opted to receive email updates will need to re-subscribe again on your new site through the Jetpack signup box, otherwise they’ll only see new posts in their Reader. Non-WordPress.com email subscribers will continue to get email updates.
Please let me know if you wish for your stats to be transferred as well.

I just came across this exchange of mails! I am facing the same issue as Swaroop. have recently transferred my wordpress.com blog Pratsmusings.com with custom domain to self-hosted wordpress blog. I need to transfer my followers/subscribers and Stats to the self-hosted wordpress. I have Jetpack installed and subscription options enabled.
Can you please do this for me as soon as possible? Please!!The blog I need help with is pratsmusings.com.
