  the annual subscription fee for the business plan is $396. Do we have to pay it every year or its a one time website creating fee and every year we have to pay for the domain name only.

  No, upgraded plans are charged annually (or for 2 years) in advance and the domain will also need paying for/renewing as well after the first year.
