Hi!
You can insert a link to the survey (you created using some other service, Survey Monkey for example) to your post or a text widget, but you cannot create a survey. You can create a poll, though:

Thanks for the reply. Unfortunately the poll button doesn’t show up when I edit or create a blog. It just says add media. It also doesn’t say add contact form either. Do you maybe know why?

Are you using the editor at the WP Admin of your blog or the new editor at your WordPress.com dashboard? The Poll button should be displayed if you are at WP Admin area. Could you upload a screenshot to your website or imgur and paste the link to it here so that I can take a look? Thanks :)

I can see that you are using the editor at WordPress.com directly. To reach the WP Admin editor, please go to yoursite.com/wp-admin. In your case, that should be:
http://diceblogsite.wordpress.com/wp-admin
If you are already logged into WordPress.com, you’ll automatically enter the WP Admin. If not, please use your WordPress.com details.
Once you are at your WP Admin, click on Posts at the menu on the left side and select to either create a new post or edit one of the existing. When you enter the New Post or Edit post page, you should see the Add Poll button, just above the editor. For more instructions on adding polls, please review https://en.support.wordpress.com/polls/.
