Swapping from wordpress.com to a wordpress self hosted site

  I want to transfer the subscribers/followers I already have on my wordpress.com blog over to my new worpress self hosted site.

  How do I do this without asking them all to subscribe again?

  The blog I need help with is: (visible only to logged in users)
