  I can’t figure out where it came from, not a plug-in on any page I can find.

  Found Table of Contents widget in Essential Addons Premium Extensions, turned it off; gone. Don’t recall using the widget anywhere in my site.

  We’re glad you got this sorted. I noticed you had a few site URLs on your account. Was this for the WordPress.com hosted site (bollandmitchell.wordpress.com), or one of your other sites?
