The 'Add a contact form' option isn't working.

  The ‘Add a Custom Form’ option isn’t working – how do I get it fixed?
  When I click the button I go to the loading screen which just hangs, if I click the loading screen I go back to my page.

  The blog I need help with is: (visible only to logged in users)
