The Jetpack comment form is not present on he page – the last form, Yes

  I have activated the jetpack comment module on my wordpress blog, but I always have the wordpress form on the page : http://www.institut-reiki-renaisens.com/valider-mes-soins/grand-programme-soins-reiki-mars/

  Could you telle me how must I do to put the jetpack form ?
