The link in my text HTML widget isn't displaying properly.

I copied the code for my MailChimp signup survey into the text HTML widget, but the code doesn’t fully transform into the proper signup form. Part of the code becomes the first half of the signup form I want, but the rest is missing. Namely, the box where people can put their email address into never appears. This isn’t the first time I’ve copied code into my WordPress blog and had it only partially work. Any tips? The blog I need help with is watermarkartcenter.org, with the widget at the bottom Left of the home page. Thanks!

The only way to have mailchimp is to use the widget Staff provide. See: https://en.support.wordpress.com/mailchimp and know that the embed code will not work on a wordpress.COM blog.
