The Premium upgrade plan applies to single blog only and is for one year only. Premium themes are not included in the Premium bundle of upgrades. They are individually priced and are a one time purchase.
WordPress.com provides free blogs and hosts them free of charge. There are no bandwidth charges. All WordPress.com blogs come with 3000 megabytes (~3 GBs) of space for storing uploaded files and images. Free features are listed here https://en.wordpress.com/features/

Sorry one more question. The package I bought in December was the premium package for one year. ($99). I got the unlimited premium package for free ($120) as a special for one year.

Premium themes and premium bundles are two different things entirely. One is a bundle of specific upgrades: the other allows you to use any premium theme on your blog. http://store.wordpress.com/

Yes, as raincoaster explained, the WordPress.com Premium plan gives you access to advanced blog features for one of your sites for one year.
The Unlimited Premium themes package gives you access to all of the Premium themes on WordPress.com for a year
