Theme for food, the price of the food depend on the weight

  Hi,

  I want to make a site for homemade food for dogs. The price will depend on the weight of the dog.
  I need a theme where the customer can add a name of the dog, weight of the dog, activity and finally he will have 3 recipes and when he choose on of them, he will see the price of the food for 1 week. And the price is different when the dogs weight is 3 kg or 25 kg for example. Is there any plugin like this? I have the formulas for this and I can integrate them in the plugin or I dont know.
  And it will be perfect if the theme has a form where the customer can add his information for the delivery.
