  You can do this yourself by following the step by step instructions here:

  Or WordPress can take care of it all for you through a Guided Transfer
  (which includes a one-time fee):

Depending on what features you are looking for in a self-hosted WordPress site, I would recommend checking out the WordPress.com Business Plan which allows for plugins and custom themes to be installed right here on WordPress.com.
