If I want to upgrade, can my followers be transferred over. Including the ones that follow me through reader. Or will they have to re-follow?

What do you mean by upgrade? Which one? There are many upgrades of different kinds. They are all listed here > http://en.support.wordpress.com/upgrades/

Do your mean you want to hire a web host and set up a WordPress.org install and move you content and subscribers and stats? If so, that can be done.
WordPress.com and WordPress.org are completely separate.
1. Hire a web host http://wordpress.org/hosting/
2. Set up a WordPress.org install and equip it with a theme and plugins.
3. Purchase a site redirect upgrade from wordpress.com
http://en.support.wordpress.com/site-redirect/
4. Export your content out of your WordPress.com blog http://en.support.wordpress.com/export/ and import it into the install.
