Transfer my blog followers from WP to self-hosted WP

  Hi. I need help moving the 609 subscribers to my blog to the new self-hosted website blog by the same domain name which is http://www.JanDeelstra.com. Can you please assist me with this transfer? I think I have tried everything short of giving up.

  The blog I need help with is: (visible only to logged in users)

  Hi Jan,

  I was able to transfer your your WP.com Subscribers (125) and your Email Subscribers (24) from your WordPress.com to your self-hosted WordPress.org blog. Please note that the other subscribers are from Tumblr, Facebook and Twitter. I could not transfer those.
