Transfer subcribers from wordpress.com to self-hosted blog

  Hello ,

  I have recently moved my blog from http://www.myweekendkitchen.wordpress.com to http://www.myweekendkitchen.in (wordpress org site). I have installed the Jetpack plugin and connected it with my WP.com account.

  Please help me transfer my subscribers from the WP.com blog to my current site.
