Transfering followers to my self hosted blog

  Hi, I would like to transfer my followers from wontbefixed.wordpress.com to my new self hosted version of this blog found at wontbefixed.pl.

  I already installed jetpack plugin there.

  The blog I need help with is: (visible only to logged in users)

  Please confirm that you have installed JetPack so staff can make the transfer for you.
