Transferring blog subscribers

  I need help transferring my blog subscribers from my WordPress.com blog to a self hosted site using the WordPress platform. Is this possible? I would need them transferred from sensiblyinspired.wordpress.com to sensiblyinspired.com and I have Jetpack installed and activated. Is there anything else I can do on my end?
