Transferring readers/ subscribers

  HI, i am sorry if i am re-writing this. but I am not sure where my first one went.

  I need help transfering past subsrbiers & FOLLOWERS from wp.com to my .org & redirecting them.. ive tried everything and different plug ins… help please!!
