Transferring subscribers from WordPress.com to self-hosted blog

Hello,
Can someone help me transfer my followers and subscribers from WordPress.com to self-hosted blog? I’ve installed Jetpack.

Jetpack does not appear to be installed, or you have connected it to a different WordPress.com account.
Have you installed, activated, and connected Jetpack? If so, which WordPress.com account did you connect it to?

Where would I go to check which account it’s connected to? Although, having said that, I think I’ve only got the one…

I don’t see any trace of wordstogoodeffect.com in our system.
If you hover over the “Connected to WordPress.com” button, it will change to a disconnect button. Hit the disconnect button, then reconnect to your wordstogoodeffect WordPress.com account.

OK, I’ve done that. I had my own domain when the blog was at wordpress.com – will that make any difference (ie the address was wordstogoodeffect.com). It’s still there at
http://wordstogoodeffect.wordpress.com/wp-admin/ -
It won’t make any difference, but I do see the blog now and have transferred your subscribers as requested.
