Hello there! Is your self-hosted site running WordPress as a blogging platform? If so, all you have to do is install Jetpack, connect it to your WordPress.com account, and I can move your subscribers for you!
Here’s some information about how to download and install Jetpack on your server. It’s free, just like WordPress, and has a simple installation process.

Our self-hosted site is not running WordPress as a blogging platform. We are using feedburner for our new subscribers.

An automated is not possible, I’m afraid. My suggestion is to create a WordPress.com blog post that will be delivered to all your subscribers. Encourage them to sign up for Feedburner. This is a move your readers will have to make manually.
