Trying to Fill out the Site Archived/Suspended Form

  I am working to fill out the form as recommended on a previous thread that has since been closed out. The website is required, as I am filling out the form in its entirety, and trying to submit it wil lnot let me submit and says that URL needs to be input. The URL IS filled out? I have verified it is the correct URL …
