Trying to find a lead generation/referral plugin

I’m interested in moving to a site over to WordPress, but have a fairly complicated form that either needs to be replaced, or be migrated over. The form works by allowing the visitor to enter their postal code; then displays 3 of our closest contractors, and emails the visitor’s information to the contractors. Allowing the contractors to contact the visitor to find out specifics regarding their project.
My searches have come up empty-handed. Are there any plug-ins available that do something similar or is this something that would need to be custom built?

If you are reading anything that makes you think that we bloggers can install plugins, custom themes or third party themes on sites hosted by wordpress.COM, then you are mistaken/misinformed. We have never had that ability. You are confusing wordpress.COM hosted blogs and wordpress.ORG installs on paid hosting.
Blogger installed plugin capability does not exist on WordPress.COM blogs and there is no upgrade you can purchase here that changes that reality. Permalink structure cannot be changed on sites hosted by wordpress.com
For that type of functionality, you’d need a self-hosted WordPress.ORG software install running on a paid web hosting service.

https://move.wordpress.com/WordPress.COM and WordPress.ORG are completely separate and have different username accounts, logins, features, run different versions of some themes with the same names, and have separate support documentation and separate support forums. As you are referring to a site that you want to install plugins on, if you don’t have a username account at WordPress.ORG account, then click http://wordpress.org/support/ and register one on the top right-hand corner of the page that opens, https://wordpress.org/support/register.php so you can post to the support forums there.

Thank you for your help and clarifying the difference between the two for me. I believe you’re correct, I’m looking more for WordPress.org.
