Unable to embed form on website

    Hi team,
    We had an Acuity form embedded on our website that disappeared in February this year.
    We have spoken with the Acuity team and they advised it must be an issue with WordPress.
    Can you kindly advise why the form is not showing on our website anymore?

    WP.com: Yes
    Jetpack: No
    Correct account: Yes
