Unable to generate contact form and subscriber form

  My blog Shayarana Safar (Shayaranasafar879462485.wordpress.com) is on a free plan and I’m unable to generate contact form and subscriber form of mailchimp or other than mailchimp too.

  The blog I need help with is: (visible only to logged in users)

  To help users subscribe to your site we also have:

  1. the Follow Blog widget, for WordPress.com users to follow you through their Reader, or have non-WordPress.com users follow your site by email

  2. the MailChimp widget which allows you to have a mailing list, and invite users to sign-up for it.
