Unusual error inserting images

I have an unusual issue when inserting images into a post. It used to work fine, but has suddenly changed.
If I post an image with the text view selected, I see this code, which is the correct code:
However, if I then preview the page or switch to visual view and then back to the text view, the code has completed changed to this (which is incorrect)
It also creates this incorrect code if I insert the image while in visual view.
Up until recently, it was inserting images with the correct code but has suddenly changed; it’s really odd.
Any ideas?
