Updating publicize stats

I have recently changed from http://www.selinamellon.wordpress.com to http://www.autismcreative.me by buying my domain name. All seems good apart from the Facebook & Twitter counts for individual posts has reset to 0. Is there a way I can get this back on my new URL?
