Hello all!
I am wondering, is it possible to upgrade from example.wordpress.com to example.com by paying the 18$ fee, or must I also upgrade to a premium account?

Domains are no longer available for purchase individually. They are included with WordPress.com Premium. A great way to go if you are not interested in WordPress.com Premium is to look into setting up a self-hosted WordPress.org site through a hosting provider such as Bluehost, GoDaddy, or Pressable.
