upgrade to business plan

  I want to upgrade my wordpress blog to the business plan as soon as possible. I need something that can facilitate a FORM. Customers have to provide me with specific information in order for me to know which product to send them. Please advise whether the ecommerce program allows for this – or whether there is a plugin or some other way to deal with it.
