This may be a ridiculous question but here goes. If I upgrade and become http://sicilyislandhills.com will people be automatically redirected to the new address?

If you buy the WordPress.com domain upgrade, people visiting your blog will see the domain you’ve purchased, not your yourusername.wordpress.com address. http://en.support.wordpress.com/domain-mapping/ Your blog is still hosted by WordPress.com and you’re still bound by the ToS here.
If you are leaving WordPress.com to host your blog on your own webhosting service, then people arriving at your WordPress.com address will only be redirected to your new domain if you also buy the upgrade to redirect them here.http://en.support.wordpress.com/domain-mapping/redirecting-your-blog/
