uploaded photos suddenly not seen in Browse All

I’ve been using WordPress for a few months now, and have been going back through my archives, re-uploading photos as original size instead of thumbnails. All was well till I hit an entry in May, and some of the photos on Browse All didn’t load properly. I tried to re-upload them and they didn’t appear in Browse All. I went to Browse, and there were a bunch of pale blue boxes with the photo title in it, but no option to send the photo to the editor, as a thumbnail or otherwise. I tried deleting a couple of these, and now photos I recently uploaded that are still visible in the blog also have a pale blue box with their file name. There are also blank blue boxes that say “undefined” when I click on them. I can’t seem to delete them. I appear to only have used 1% of the 10GB of storage I have, so it’s clearly not a space issue. Is the server I’m on corrupt or something?

Do you have a blog address and a couple of examples that we can take a look at? As well, have you tried reporting this to staff yet?

The server you’re on? Are you hosted at WordPress.com? If not you need to be at WordPress.org, in accordance with the sticky thread at the top marked Read Me First.
If you are on WP.com, I’ve noticed this as a sometimes error. It’s very weird; comes and goes. If you can click to Edit on the upload box, you can grab the URL and use that just fine anyway in the visual editor Image icon, or at least it worked for me last night. It fixed itself after fifteen minutes for me at least, and I didn’t do anything.
