Uploading new theme

  I’m try to uploading a new theme in my blog http://withmein.wordpress.com/ and I’ve already downloaded the latest version of Wp as request.
  I’m now in the phase where it’s asked to fill my Database Name, Username, Password and Host. From where I get these information? Are the same one of my blog or should they be different?

  I’m trying to do it by myself but there’s always a next step where I’m not sure that I’m doing the right thing.
