URGENT! Image link not working. Is there a bug in the latest wordpress release?

The code <img src=”https://www.mywinepal.com/images/4_Star_Rating.jpg” alt=”4 stars” align=”bottom” style = “border:0px”> no longer works.
This text shows up instead of my “stars”. I use the plugin CM On Demand Search And Replace. I have a short code, e.g. [usr 4.5] that when someone opens a post that has it, CM On Demand Search And Replace on the fly converts the short code to <img src=”https://www.mywinepal.com/images/4_Star_Rating.jpg” alt=”4 stars” align=”bottom” style = “border:0px”>

I should note for the example above the short code would be [usr 4] or [4]. Both would generate:
