  Hello… Our company is new to this subscription and in order to purchase we would need to have a W-9 form from your company to establish a State Vendor. Who can I forward the W-9 Form so that it can be completed?

  We do not have an account at this time but in order to make the purchase, we would need to have the company on file for our Vendors… Can you verify the information: Mail code : T26….. Taxpayer Number 32071375243… Zip 78642…
