website for speakers

I am trying to develop a speakers bureau of innovation and tech commercialization experts for a statewide committee I run in California (I also need to build a similar site for inno/tech consultants).
I want to be able to have a potential speaker fill out a form that feeds a database (like I could distribute a link to my partners to gather speaker information). The form needs to capture the speakers name, a picture, possibly a video sample, information about fees, whether they are affiliated with one of our partner organizations, and his/her area of expertise in a way that is searchable in different ways (by topic/expertise, geographic location, cost).
Thumbing up/down speakers would also be great.
I need to be able to review the information before it goes live.
I want the database to populate a website so that anyone looking for a speaker can easily find one and that all our organizations can use this centralized site. I don’t know how you reach back out, but if you can send me an email with your response to (email visible only to moderators and staff), that would be great. -
