What do I do to get my current blog onto wordpress.org with my own domain?

I’m sorry to say that I have read many of what I think are the pertinent articles you have posted and still am unsure as what to do in what order. I have a blog on wordpress.com. I want to get my own domain name and move to wordpress.org. HOWEVER, I don’t know if I have to have the 3rd party host server lined up AND what about my home page? Do I have to have a new one designed before I can move over to wordpress.org/new host server? Do I need to notify anyone like readers of new address or will it auto connect to the new domain. SORRY I hope these answers are not in the FAQ – really I did try to understand.
