What form widget includes persons name in subject line

Hi, I cant find a form that does what I want. At present the subject line can have pre determined text I add for when the email comes to me. What I want is the form to take date from one of the fields (name) and add it to the subject line. This will make it easier to find specific persons email. At present they all say web enquiry. What’s the name of the form widget or plugin that will do this please? Thanking you in advance Mark UK

The only form we can use on a wordpress.COM hosted site is a constact form and is described at https://en.support.wordpress.com/contact-form/

Im not sure if my question is written wrong. I have a WordPress site. I’m using “Visual Form Builder” Widget. Unfortionatly this widget is basic and only allows me to put text in the subject field manually. What I want to happen is when a person fills the form in on my site. The form takes the name and includes it into the email that’s sent. For example Subject line on the email in my inbox would say “Email enquiry (James Smith)” If I had 200 emails from my site in my inbox I would have to click on everyone trying to find James Smiths message. I need a form widget that allows me to set parameters for this. I dont do coding so it has to be in built into the form thanks

I’m using “Visual Form Builder” Widget.

You cannot use that widget on any site hosted by wordpress.COM – period. No upgrade changes that.

We provide support only for wordpress.COM hosted sites. Our support docs do not apply to
    (1) local installs of wordpress.ORG software on your own server or
    (2) wordpress.ORG software installs on paid hosting, and we do not provide support for them at wordpress.COM.

Also, note that we do not provide Jetpack support https://jetpack.me/support/ for sites linked to wordpress.COM accounts with the Jetpack plugin.

You are posting to the wrong support forum.

WordPress.COM and WordPress.ORG are completely separate and have different username accounts, logins, features, run different versions of some themes with the same names, and have separate support documentation and separate support forums. Read the differences here http://en.support.wordpress.com/com-vs-org/

As you are referring to a site that is not hosted here, if you don’t have a username account at WordPress.ORG account, then click http://wordpress.org/support/ and register one on the top right-hand corner of the page that opens, https://wordpress.org/support/register.php so you can post to the support forums there.
