To move your content you need to create an XML export file of your content. It will not include the theme as they cannot be exported. See 3 below.
Also note that if you have purchased a premium theme you will have to contact the theme designer and request a wordpress.org version of it.
WordPress.com and WordPress.org are completely separate.

NOTE: WordPress.com and WordPress.org have different log-ins and run different versions of themes with same names. If you don’t have a username account at WordPress.ORG click http://wordpress.org/support/ and register one on the top right hand corner of the page that opens, so you can post to the support forums there and receive advice from WordPress.ORG bloggers.

I normally develop self-hosted sites, but have a client I think would be best served now and for the coming year or two with a WordPress.com site and the $299 Business Plan. Before I suggest this route, I need to consider whether we’d be starting over (except for content) if/when the client requested a Guided Transfer to set up a self-hosted site.
I saw that premium themes don’t transfer to self-hosted sites. Do any themes transfer, or are there themes that can be repurchased for a self-hosted site? Does any Custom CSS transfer?

No themes found here either free or premium can be exported period.

NO. CSS editing is theme specific editing and cannot be transferred.

In the event that anyone stumbles across this thread in the future, I wanted to add that I’ll be recommending WordPress.com for the aforementioned project. We will be able to recreate essentially the same – if not exactly the same – site in a self-hosted WordPress site.
The theme I was considering is indeed available for use with self-hosted WordPress. Scraping the CSS out of the custom editor on the .com site and pasting it into a text document for safekeeping during the Guided Transfer should enable its reuse on the new site, preserving any style modifications made to the theme on the .com site.
With premium upgrades, the availability of a .org version of the theme of choice and the ability to either export or use Guided Transfer, I see virtually no downside for this client. The recent addition of e-commerce options may mean .com works for this client even longer than anticipated.
