Im trying to put a survey from vizu, but where is the template to start with?
or do you have to upgrade to do that?

Wordpress.com is a multi-user blogging platform and we cannot edit our templates here. If we were allowed to then the changes you made to your theme would be made to all other bloggers using the same theme. We can purchase an upgrade that will allow us only to make appearance changes but not to change functionality.
Because this question frequently arises I have prepared a blog post that addresses the subject in depth. This thread will show you the 2 choices available for personalizing a theme at wordpress.com as well as the further choices of either self-hosting a blog on your own server or hiring a web host to do this for you.
Wordpress.com and wordpress.org run on different software and it is possible, if you wish to have full control over your template, to hire an independent web host and have him or her download a free blog template from http://wordpress.org and edit it to suit your needs.(2) Re: Survey
It’s not possible to answer this question without seeing the survey in question.
