While using migration tool, I get “Unable to load manage page for this blog"

While using the subscription migration tool, I get an “Unable to load manage page for this blog” message after pressing “manage”. BLOG: rprtphoto.wordpress.com. SELF-HOSTED SITE: rprtphoto.com.
The blog I need help with is: (visible only to logged in users)

Just to be clear—I need to transfer my WordPress.com followers and email subscribers at “rprtphoto.wordpress.com” to my Jetpack-connected self-hosted WordPress site at “rprtphoto.com”. I will also post a notice on my old site about my upcoming move to keep my readers informed.

I’m having the exact same issue… I’m trying to use the tutorial offered by Jet Pack but get the response “Unable to load manage page for this blog”
I’m trying to take my followers from makeanddohandmade.wordpress.com over to my new url: makeanddoblog.com
Please help!!

Jetpack’s subscription migration tool says that “Keep in mind that while your email subscribers will continue to receive email notifications of new posts as before, WordPress.com followers will only see new posts in the Reader. They will not receive email updates unless they subscribe to receive those on your new site. You might want to post a notice on your old site about your upcoming move to keep your readers informed.”
I’m assuming that a simple post to that effect is what is meant?

Five new followers have shown up on the old blog. Will they auto-migrate over to the new website? [It doesn’t look like it] Please give me instructions on how to take care of this myself instead of doing it for me?
