Why am I now asked to confirm each and every comment?

Before when I followed a blog or left comments I was asked only the first time. Now each comment I leave requires me to confirm it (by email) How can I turn this off?

Hi there. What exactly do you mean by having to confirm each comment? Do you have to enter your email in the comment form each time you comment?
If that’s what’s happening, the site might not be keeping track of your login correctly and treats you as if you were logged out when you try to leave a comment on someone else’s site. Please clear your browser’s cache and cookies and make sure that you have third-party cookies enabled: http://en.support.wordpress.com/browser-issues/
http://en.support.wordpress.com/third-party-cookies/Also note that if you have any browser add-on installed that automatically logs you out of websites when you leave them, that might be causing this problem. Adding WordPress.com to that add-on’s exceptions/whitelist (if possible) should solve the problem, but if there isn’t such an option uninstalling the add-on might be the only solution.
If your problem is something else entirely, please try to provide some more details on exactly what is happening.

I am unfamiliar with clearing cache and cookies but think I have done so. I have recently added comments to a new site and each time I get an email asking me to ‘confirm follow’ and older sites I have followed for ages are now asking the same thing. On my own manage subscriptions page each of these comments seems to be a separate subscription, now six to one site!

Ah, okay. You’re signing up to follow the comments as you make them. When you make a comment, check the little box at the bottom that says Notify me of follow-up comments via email. Make sure that’s unchecked if you don’t want to receive an email every time anyone leaves a comment on that post.

That might be ambiguous. Let me try again.
When commenting, make sure that box is unchecked. If it is checked, you’re subscribing to comments. I hope that’s more clear :)
