Why are comments not appearing in comment list?

  Many comments are entered on the blog page by unverified WordPress users (they enter a name and email) and I get a comment notification email, but they do not appear on the comment list either as spam or waiting for moderation. The blog is set to require moderation on the first comment. There are probably over a hundred cases of this in the past few days as one of my blog posts seems to have gone viral.
