Why aren't all my placeholder images displaying?

  My autofocus-themed blog shows placeholder images for certain images but not for all. I don’t want to use featured images as I have then two images in each post. My blog is purely for images so I’d like my front page to show placeholder images of each post. Is it to do with the size of the images? Help!
