Why aren't my moderation settings working as expected?

I have my comment moderation settings set as shown here:
http://en.support.wordpress.com/settings/discussion-settings/#before-a-comment-appearsI want it to behave as described there, which is (1) to notify me (on the mobile app and/or email) of every comment, but (2) to only require moderation approval for first-time commenters.
(1) is happening, but instead of (2), it is requiring my explicit approval for every comment.

You could try the old “universal config fix” – change the config – save changes then change back to what you want and save changes again – once in a while things will get out of sync somewhere

timethief, this is not an app issue. I am getting “Please moderate” emails for every comment (including from repeat commenters).

Hi @ratamacue0,
It looks like your current Discussion settings are set to notify you every time someone posts a comment. However, new comments will automatically be approved provided the commenter has already had one approved comment. Can you confirm that these notifications you’re receiving through email are “Please Moderate” emails, not “Comment: …” notification emails?

They are “Please moderate” emails.
I haven’t been actively posting on my blog since I posted this help request. In the meantime, one particularly active commenter has since become able to comment without requiring moderation. Other commenters haven’t been so active, so I don’t know if the problem has gone away; or if it persists, but potentially self-resolves after many comment approvals, instead of one (as I believe it should).

I wasn’t able to recreate this on a test blog. When I approved one comment from a commenter, I did not receive any additional “Please Moderate” emails from that individual. Can you let me know if this persists after your next post on your blog?

I made a new post. The issue persists–with commenters who have commented on other posts, and with the commenter who made two comments on the new post, in a single thread (only one so far).

For the first comment here, I believe this was held in moderation because it contains 2 or more links (per the option in your Discussion settings).
However, these two comments should not have been held for moderation:
Can you confirm that you did receive moderation emails for these two comments?

Indeed, I did receive moderation emails for all comments on that post that were not my own.

Also, I got the following message from one of my followers. I don’t know if there’s any relation to this issue.
“I’m not sure why but for your blog the comment subscription seems to be very buggy.
It almost seems random as far as which comments I get e-mails on and it seems most
of the comments do not get sent. This shouldn’t be too big of a deal, but if someone
later tries to comment in reply to me then it may be that I will never know.”What is wrong with my blog? Could these issues be theme-related?

Hi there,
We’re still investigating this issue. If possible, could you send over the email address of the individual having trouble receiving comment notifications? Just a note, the address will appear redacting within the forums and will only be visible to staff.

It looks like she is receiving comment notifications and received them for your latest public post “To Christians: A Warning Plea”. However, she will not receive notifications for password protected post comments.
